Can you help me with concept questions?

I'm currently doing my CELTA training course and have been given and assignment to do. I'm OK with most of it except concept questions. I have two sentences that i need to generate concept questions for;

Karen wishes she'd worked harder. (the target language is underlined)

The two concept questions i have come up with so far are;

Is Karen pleased?
Did she work hard enough?

But i cant for the life of me come up with a third.

John's visited India three times

Again, i have come up with two so far but need a third;

Did John go to India?
Is he there now?

Any help with these two sentences would be greatly appreciated.

You've come up with some good ideas. I suggest the following additional questions.

Karen wishes she'd worked harder.

Does she regret that she didn't work hard enough?

John's visited India three times

Do we know when John went to India? In answer to this, we might know.
