Teacher register

Hello fellow English Teachers. I've got a question. I come from Poland and a lot of schools in our country still use teacher registers in a book form (although some use electronic ones too). Tell me please, how do you fill in the categories/rubrics in our subject - English (I mean e.g. reading/tests/quizzes/speaking, etc). And one more question, how do you call a task in which a chosen student reminds other students of what language material was covered during the last lesson or three last lessons and then is given a grade for his/her performance.
