Recording of lexical items in note books

Hi there people,

Does any experienced teachers know of alternative way of recording lexical items in a note book for beginner and intermediate level learners. I found the following diagram, which is very useful. I wish to broaden my knowledge and ability as a teacher. Ideally 2 or 3 alternates would be good.
