Continuous assessment record

Hi! I'm new to this forum and I hope you can help me!

I am preparing my public examination to work as a state teacher of Official Schools of Languages in Spain. We have to elaborate a teaching plan for one academic year and also we have to take a written and an oral exam. In our oral exam we have to defend our teaching plan and exploit one didactic unit of that teaching plan. Well, in my teaching plan I talk about the lesson-planning sheet, for the teacher, the continuous assessment record (or sheet or form), for the teacher to have a record of students' performance in the classroom, and the self-assessment diary (or sheet or form) for the students. The problem is that I don't have any of these documents so that I can include them in my teaching plan to show what I am talking about, and would like to know if there's anywhere where I can find something similar to elaborate these sheets.
