I used the VISTA imaging system for scanning medical and administrative documents such as advance directives, lab reports, health benefit applications and patient documentation into Veterans' medical records in a timely manner. My productivity rate was approximately 100 documents per day, with a quota of 80 documents per day and a maximum of eight errors per month. Before scanning I sorted all documents by priority category. I organized archived files in storage.

I handled phone requests from Release of Information or other VA facilities to pull paper records or documents at the request of veterans or their families for medical, legal, benefit claims and other purposes, either hand-delivering, faxing, or sending copies of correspondence via interoffice mail; took phone requests from Veterans who need records transferred, providing them with the necessary information that they need in order to make a smooth transition, such as registration with their new facility, and answering any questions or addressing any concerns that they had; used the VISTA system to track records at the request of individual veterans, Release of Information, Admissions, and units throughout the Hudson Valley Health Care System and outside VA facilities, using locator cards to track inactive records; transferred records from the home facility to other facilities using the Federal Records System; charged out and checked in records that were forwarded to Release of Information at the request of Veterans or their families; received, transferred and routed incoming calls and ensured that patient confidentiality and HIPAA regulations were followed properly at all times.
